Medical Associates Health Plans (MAHP) has developed a network, which includes local primary care practitioners and specialists as well as local hospitals and other providers of medical care. The Plan maintains a stable, consistent network of providers throughout the service area for members to utilize without a referral. The Plan issues a referral to tertiary centers or other specialists and hospitals when care cannot be provided within the local network.
 
Many providers are now offering virtual visits (telehealth) - please call their office to ask about options.
MAHP updates the directories daily, please call Member Services at (866) 821-1365 or (563) 584-4885 for an updated directory.
 
If you receive charges from an out-of-network provider who you believe was listed as in-network in this provider directory at the time of service, you may either file an appeal with Member Services by calling 563-584-4885 or sending an email to MAHPMemberservices@mahealthcare.com. See the Complaints, Appeals, and External Review Procedures section of your Subscriber Agreement for more information on filing an appeal. You may also file a complaint with your state’s Department of Insurance.
